Prescription Refill 

​​​​​To obtain a prescription refill, please call our offices during routine business hours, Monday - Friday 9a.m. - 5p.m.

When calling, please have your prescription information, including your pharmacy phone number readily available. We will attempt to honor your refill requests within 2 business days of your call.

 

Financial Information 

In order to accommodate the needs and requests of our patients, we have contracted with numerous managed care companies. By doing so, we agree to file your insurance claim in a timely manner and to accept a discounted fee fro service, in addition to fulfilling other contractual obligations. It is your responsibility to contact your insurance company to verify that we are on your particular plan.
